MINDFULNESS CENTER AT BROWN REFUND POLICY
Mindfulness Center at Brown will make every effort to hold classes as scheduled. When the unforeseen happens, the Center reserves the right to cancel a program at any time. If Mindfulness Center at Brown cancels a program, you will receive a full refund.
Refunds are available (less a $50 processing fee) up to 14 days before your program or stay begins.
Non-refundable credit toward a Mindfulness Center at Brown future program (less a $50 processing fee) is available if you give notice between 13 and one day(s) before your program begins. Credit may be applied to any Mindfulness Center at Brown program for one year following date of issue, if you meet the prerequisites for that program.
No credit or refund is available if you cancel on the day the class starts; if you do not show up; or if you leave a program early for any reason. No refund will be available if you attend a program and are dissatisfied with its presentation or content.

MINDFULNESS CENTER AT BROWN PHYSICAL ACTIVITY ASSUMPTION OF RISK AND RELEASE AGREEMENT
I am a participant in a mindfulness training program offered by The Mindfulness Center at Brown and have voluntarily chosen to participate in the program above. This Assumption of Risk and Release Agreement (“Agreement”) confirms my understanding of and agreement with the following:
1.I understand that participation in the Program involves a risk of physical injury to me. This includes without limitation risks that may result from my participating in the mindful movement components of the program including scratches, bruises, sprains, strains, burns, tears, broken bones, concussion, respiratory or heart failure, spinal injury, paralysis, stroke, and even death. I have made my own investigation of these risks, understand the risks and assume them knowingly and willingly.
2.In consideration for being allowed to participate in the Activity, I release indemnify, and hold harmless Brown University, including the Corporation, its Trustees, Fellows, officers, employees, representatives, and agents, from and against any present or future claims, losses, liabilities costs, and expenses for personal injury, including death, property damage, or any other damage, which I may suffer, or for which I may be liable to any other person, related to my participation in the Program.
3.I affirm that I am physically and mentally capable of participating in the Program and have no known health restrictions that may jeopardize my health or safety while participating in the Program. I understand that it is my responsibility to inform the Responsible Staff Member (Teacher for the Program) listed above of any health conditions that may limit my ability to participate in the Program. I agree to cease participating in the Program if I believe further participation poses a risk to my health or safety. In the event of illness or injury, I authorize the Responsible Staff Member listed above and other agents of Brown to coordinate emergency care or other medical treatment for me based on the existing circumstances.
4.I understand that my participation in the program is subject to all policies, rules, and procedures of Brown, the site of the Program, and/or as outlined for me by the Responsible Staff Member listed above.
I certify that I have read and understand this Agreement and am at least 18 years old. I understand and agree that any oral or written representations not contained in this Agreement will not alter the content of the Agreement. I agree that this Agreement shall be governed by the laws of the State of Rhode Island, excluding its conflict of laws principles, and that the federal or state courts in the State of Rhode Island shall be the forum for any lawsuits filed under or incident of this Agreement.
